Located in Erie, Pennsylvania, Community Health Net (CHN) is a Federally Qualified Health Center whose mission is “to improve the quality of life in our region, by providing professional healthcare services, with compassion, respect, and dignity to all.”  CHN is celebrating 38 years of service to the residents of Erie County.   CHN is a multi-specialty practice providing in addition to Family Medicine, Internal Medicine, Pediatrics and Infectious Disease.  CHN is JCAHO certified and strives to assist its patients towards a healthier lifestyle.  In addition to Primary Medical Services, CHN offers health education, social services, OB/GYN services (by referral), Vision, free transportation, Health Care for the Homeless and Ryan White HIV/AIDS medical services.  CHN accepts all patients regardless of their ability to pay for services and offers a Sliding Fee Scale to all income eligible uninsured and underinsured patients based on household income.  CHN has on site a 340B Pharmacy that provide discount drug pricing for patients are each of its 3 medical and 3 dental locations
