
Southcentral Foundation
Southcentral Foundation, an Alaska Native-owned, and nonprofit health care organization serving nearly 65,000 Alaska Native and American Indian people living in Anchorage, Matanuska-Susitna Valley, and 55 rural villages in the Anchorage Service Unit. Some of our funding comes from the Indian Health Service, but we are our own tribally managed non-profit corporation. Since we are affiliated with IHS, individuals that work for us will qualify to apply for Federal Loan Repayment, and are covered by Federal Tort.
In 1999, Southcentral Foundation and the Alaska Native Tribal Health Consortium signed an agreement to take over ownership and management of the entire Alaska Native Medical Center (ANMC). Southcentral Foundation primarily operates the outpatient programs. Most sub-specialties are available across the street from our Anchorage Native Primary Care Center at ANMC with an excellent referral process in place to help get customers the care they need.
Our Anchorage clinic is surrounded by majestic mountains and houses primary medical care, dentistry, optometry, complementary medicine, physical therapy, fetal alcohol syndrome prevention, substance abuse treatment, domestic violence prevention, diabetes prevention, suicide prevention, residential treatment for pregnant women, residential treatment for adolescents, psychiatry, mental health counseling, traditional healing, home health, health education, youth mentorship and Elder programs.
Our Wasilla clinic is located in the beautiful Matanuska-Susitna Valley, 45 minutes north of Anchorage. It is a brand new facility that houses a primary care center, optometry, audiology, physical and occupational therapy, dental, pharmacy, radiology, and a behavioral health center.
Southcentral Foundation’s Nuka System of Care has been recognized as one of the world’s leading models of health care redesign and is a recipient of the 2011 and 2017 Malcolm Baldrige National Quality Award. Our award-winning model of care is based on our belief that the journey to wellness is a responsibility between patient and provider.
Nuka is built upon the simple, yet revolutionary belief that the relationship between the primary care team and the patient (known as the customer-owner) is the single most important tool in managing chronic disease, controlling health care costs, and improving the overall wellness of a population. Recognizing that individuals are ultimately in control of their own lifestyle choices and health care decisions, Nuka focuses on understanding each customer-owner’s unique story, values and influencers in an effort to engage them in their care and support long-term behavior change.
