For many of us at Community Care Physicians (CCP), taking care of people isn’t just the career we have today; it’s the passion we’ve had our entire lives. We are proud to be an independent multispecialty medical group in the Capital Region, NY - and we are truly independent. Although we pride ourselves on our relationships with the many hospitals and health plans in the region, we are owned and governed by our physicians; physicians of multiple specialties who are all practicing medicine and have a pulse on the front lines of healthcare.
CCP’s business model is comprised of decentralized clinical and operational decision-making at the practices level supported by a central administrative infrastructure working in partnership with the practices to relieve the physicians of administrative burdens and to gain economies of scale. The highest priority of everyone at CCP is to deliver top quality and efficient health care to our patients and doing so in a manner that promotes professional satisfaction with our physicians and staff.
CCP is built on relationships – relationships with our patients, our colleagues, and our communities. CCP has over 30 different specialties within the organization and touches every three to four people in the greater Capital Region. Our extensive primary care network of over 40 offices serves as a portal of entry into Neurology and our other integrated subspecialty offices. As one organization, our practitioners are connected and regularly interact to share best practices, develop optimal processes and innovative ideas, teach each other, and create the best experience for our staff and patients.
CCP’s Neurology Division specializes in both adult and pediatric neurology so we can provide care to a wide range of patients, from newborns to geriatrics. Our neurologists are highly trained in treating neurologic conditions such as neuropathies, concussions, and migraines. They are also able to treat patients who have been diagnosed with neurologic diseases, to include Epilepsy, Parkinson’s, and Alzheimer’s. We also possess state-of-the-art, on-site neurology laboratory facilities staffed with our own certified technicians where we can perform EMG and EEG studies.
