Valley Medical Group is seeking to hire an experienced Physician Assistant or Nurse Practitioner for our Family Practice department at our Greenfield, Amherst, Northampton and Easthampton sites.

- Services include primary care (Family Practice, Internal Medicine, Pediatrics), PT/OT, Behavioral Health and Psychiatry, Optometry, Podiatry, Rheumatology, Endocrinology, Sports Medicine, Diabetes and Nutrition Care

- We have long experience in population health having been part of Kaiser Permanente until 2000
- Our unique practice model uses a state of the art electronic health record and outreach systems to offer patients same day appointments, and immediate access to care that is highly coordinated and focused on achieving the best possible health outcomes
- We have same day mammography and lab draws for improving access to services that are essential for quality goals and patient health
Team-Based care
- We work in teams for better communication and coverage
- Our teams are well supported by patient focused clinical and administrative support staff
- We support our clinicians in the following ways:
- Integrated behavioral health services available in primary care exam rooms for quick assessments and to help with emotional problems and behavioral issues interfering with medical treatments
- Controlled Substance Refill program for help with patients presenting with pain and/or addictions
- Clinical Clerks in primary care to assist with refilling prescriptions and other administrative burdens
