Assess and provide home care for patients living independently for:
- Acute and post-acute care
- COPD/Asthma
- Cellulitis
- Dehydration
- Gastroenteritis
- Heart Failure
- Altered Mental Status
Services provided:
- Remote patient monitoring
- Imaging
- Pharmacy
- Medical equipment
- Physical therapy
- Occupational therapy
- Infusion pharmacy
- EMS-Emergent and non-emergent transport
Position Details:
- PRN opportunities available for 12 -hour shifts
- Provide daily clinical guidance and decision-making as the attending Advanced Care Physician
- Collaborate with primary care physicians and offices, nurses, and nurse practitioners
- Typical Day:
- 9a- 5p for patient visits
- See 10- 12 patients per day on average, spending time with each and discussing caregiver support, vitals, assessments, etc.
- On call; available for ongoing orders, next steps, coordinating care with staff, communicating with primary care provider at the end of each day or following day.
- Answering service takes after-hours call first, then dispatched out to the team
- Coverage Areas: St. Louis, St. Charles, Jefferson County

Requirements:
- Hospitalist or home health experience preferred
- Experience and training in quality improvement and patient safety
- Able to create personal relationships and build human connections
- Communicate effectively with patients, family, medical power of attorney, primary care provider and all individuals involved in the patient s care
- Provide care with compassion, empathy, and cultural competency.
- Board Certified
- Active Missouri Medical License or ability to obtain
- Enrollment in Essence and other Medicare HMO plans
- ACLS Certified
- Good driving record
