Position Overview
HarmonyCares Medical Group is seeking a highly skilled Nurse Practitioner to join our team of healthcare professionals in the Chicago area. The ideal candidate will have expertise in providing comprehensive primary care to high-acuity geriatric patients residing within a 60-mile radius of Burr Ridge, IL.
Responsibilities
* Conduct thorough patient assessments and develop individualized care plans
* Manage small patient panels with up to one hour of time per patient
* Provide a range of services, including medication management, symptom control, and end-of-life care
* Collaborate with physicians, medical assistants, and other healthcare team members to ensure seamless patient care
* Monitor patient outcomes, adjust care plans as needed, and provide ongoing support
* Conduct home visits to assess patients' needs and provide care
* Maintain accurate medical records and documentation
Qualifications
* Master's degree in Nursing Practice (MSN) or Doctorate of Nursing Practice (DNP)
* Board certification as a Nurse Practitioner (NP-C)
* Minimum of 2 years of experience
* Strong clinical skills, including physical examination, diagnostic testing, and medication management
* Excellent communication and interpersonal skills
* Ability to work independently and as part of a team
* Valid driver's license and reliable transportation
