Family Medicine Physician Job at Central Health in Austin, TX

Job Summary:

Central Health is seeking a physician, board certified in Family Medicine or Internal Medicine to join our Pre-Operative Assessment and Medical Optimization Programs. This role requires a thorough understanding of various medical conditions, their impact on surgical outcomes, and strategies for optimizing patients' health preoperatively. The Pre-Operative Assessment program will risk stratify patients undergoing surgery or procedures requiring anesthesia. It will require communication and collaboration with specialists for complete evaluation.

Patients who have been determined to be high risk or have uncontrolled chronic health conditions during the pre-operative assessment or who are identified during a visit at the Central Health Multi-specialty clinic, will be cohorted for intensive disease management in our Medical Optimization program. As a member of our multi-disciplinary team, this will require close collaboration with specialists, pharmacists, and dietitians to provide comprehensive care tailored to each patient's needs.  Job duties include direct patient care to referred patients via face-to-face visits or telehealth. This position requires supervision of advanced practice providers, students, residents and fellows.
