Family Medicine Physician Job at Fairview Health Services in Brooklyn Park, MN 55443, MN

1. 16 provider primary care practice with more than 12 specialists onsite.
2. Patient encounters average 18-22 patients per day.
3. 34 patient contact hours per week.
4. Call Frequency: approximately 1 per month - phone consultation only with first tier nurse triage.
5. Openings: 1.0 FTE (Full Time) or less - Flexible schedule.
6. As a Pioneer Accountable Care Organization (ACO), our physicians use a multidisciplinary team approach in managing clinical care.
7. Fully integrated Electronic Medical Record (EPIC).

Qualifications:

1. Graduate of an accredited ACGME/AOA Family Medicine residency training program.
2. BC/BE with the ABMS or AOA.
3. Licensed or ability to obtain licensure through the Minnesota Board of Medical Practice.
4. Ability to obtain and /or maintain DEA certification for the State of Minnesota.
5. Meet threshold criteria/qualifications for Credentialing and Privileges if applicable.
