Job Description

Position: Full-Time Primary Care Physician (Family Medicine or Internal Medicine)

• Outpatient Only
• Join a team of 16 providers who focus on value-based care
• Market Competitive Salary with large bonus potential
• Medical Directorship Available
• Affiliated with Ascension St. Joseph Medical Center and Silver Cross Hospital
• Nursing home coverage available but not required
• Tech-forward company with cutting-edge EMR and software to assist providers in the Value-Based Model
• Patient-Centered Medical Home Level 3
• In-house STAT labs for point-of-care diagnosis
• Earning potential over $300,000
