Job Description

Licensed Practical Nurse (LPN)
Location: Rye NY
Status: [Full-Time OR Part-Time

PAY-$34.71 hourly

The Licensed Practical Nurse (LPN) provides direct nursing care and administers medications and treatments to residents in accordance with their care plans. This role supports the Assisted Living Department’s mission by delivering services with professionalism, positivity, and respect for the Resident Bill of Rights, organizational policies, and procedures.


Essential Job Functions

Resident Care Services:

  1. Provide direct nursing care based on individualized resident needs.
  2. Deliver care as prescribed by primary and consulting physicians.
  3. Assist with and supervise residents’ nutritional needs.
  4. Implement resident care plans in collaboration with the interdisciplinary team.
  5. Perform other duties as assigned.

Compliance:

  1. Gather data to address residents’ physical, psychosocial, spiritual, and cultural needs.
  2. Accurately document care and maintain accessible records.
  3. Communicate pertinent information to the Assisted Living Nurse Supervisor and care providers.
  4. Ensure residents’ rights to privacy, dignity, and individuality in all care activities.
  5. Follow all applicable State and Federal regulations, including those on abuse prevention, HIPAA, and resident rights.

Leadership Responsibility:

  1. Supervise ancillary personnel, ensuring safe, efficient resident care as outlined in care plans and guides.

Knowledge and Skills

  • Licensure: Current NYS Licensed Practical Nurse credential.
  • Certifications: Current CPR certification required.
  • Nursing Expertise:
    • Strong foundation in basic nursing care techniques.
    • Ability to assess and communicate changes in resident conditions.
    • Capacity to make independent decisions as needed.
    • Familiarity with computer systems and willingness to adapt to new care technologies.
  • Interpersonal Skills: Patience, tact, enthusiasm, and leadership ability.
  • Commitment to Learning: Eager to adopt new methods and integrate them into practice.

Physical and Sensory Requirements

  • Proficient in speaking and writing English.
  • Mobility to navigate throughout the facility.
  • Capable of bending, sitting, and standing for extended periods (up to 7.5 hours).
  • Ability to lift and carry items weighing 10–50 lbs and push/pull carts and wheelchairs weighing up to 200 lbs.
  • Ability to manage the mental and emotional stress of caregiving.
  • Functional sensory abilities, with or without prosthetics, to communicate effectively in person, by phone, and in group settings.

Communication Requirements

  • Maintain open, consistent communication with staff to:
    • Receive direction on care updates.
    • Report incidents or issues.
  • Daily verbal and face-to-face interactions with staff.
