Job Overview:

A geriatric physician or geriatrician specializes in the care of older adults focusing on the unique health issues and complexities associated with aging. A geriatrician has additional expertise in neurology, psychiatry, clinical pharmacy and rehabilitation medicine and are skilled in recognizing asymptomatic and atypical presentations of diseases in the elderly. They provide a comprehensive clinical approach by diagnosing medical conditions, assessing frailty, managing co-existing morbidities, developing treatment and follow-up plans, coordinating care, evaluating long-term care needs and providing end-of-life care ensuring quality of life is maintained to optimize patient outcomes in the aging population.

Job Description:

Clinical:

  1. Serve as a Geriatric consultant to John Muir Health patients providing expertise and recommendations for older adults with complex medical issues by collaborating with primary care physicians, specialists, and interdisciplinary teams to organize health services and optimize the care of older patients with multiple chronic conditions or acute medical issues.
  2. Conduct multidimensional assessments focusing on the 4Ms to evaluate an older person's overall health, functional ability, physical health, cognition, mental health, and socioenvironmental circumstances.
  3. Review patients' medical histories, perform physical examinations, and order diagnostic tests to diagnose and manage chronic medical conditions, acute illnesses, mental health disorders and age-related health issues.
  4. Perform medication review and management recognizing adverse drug events, reducing polypharmacy and de-prescribing.
  5. Develop personalized treatment plans that address the unique needs and preferences of each patient, including medication management, lifestyle modifications, preventive care, and coordination of specialty.
  6. Actively engage in patient education and shared-decision making to empower older adults in the management of their health outcomes.
  7. Provide consultative discussions and recommendations with family, patients, and interdisciplinary team members as needed.
  8. Collaborate closely with the interdisciplinary team and other specialists to enhance efficiency and care coordination to provide comprehensive care.
  9. Serve as the supervising physician for Nurse Practitioners/Physician assistants in the clinic setting.

Administrative:

  1. Facilitate in the design and implementation of a Geriatric Medicine Consultative Clinic with ongoing participation in restructuring of the workflows and processes to meet the need of the community.
  2. Oversee the members of the interdisciplinary team in the Geriatrics Clinic.
  3. Assist in the development of health system wide processes and workflows tailored to the needs of the elderly with a focus to manage complex conditions like dementia, falls and polypharmacy.
  4. Focus on initiatives for delivering high-quality, value-based care while maximizing patient outcomes.
  5. Assist in system wide endeavors to prepare the aging community by promoting healthy aging, preventing age-related health problems, and improving the quality of life for older adults.
  6. Provide evidence-based and national practice guidelines when implementing clinical workflows.
  7. Attend interdisciplinary and committee meetings as needed.

Qualifications:

Required:

  1. Doctor of Medicine or Osteopathy from an ACGME accredited medical school
  2. US licensed Physician (California license preferred) with Board Certification in Internal/Family Medicine.
  3. Successful completion of an ACGME or AOA accredited Geriatric Medicine fellowship program.
  4. Must provide references focusing on quality, excellent patient centered-care, bedside manner and professional demeanor and collaboration in multidisciplinary team environment
  5. Must be physically able to perform all required duties in the clinic setting.
  6. Adhere to evidence-based, national and internal practice guidelines and policies.
  7. Comfortable taking care of frail older patients with multiple comorbidities and demonstrate empathy, compassion, and respect for the unique needs of elderly patients
  8. Excellent communication skills with ability to adjust communication styles when working with patients, their families and colleagues.
  9. Flexibility and willingness to pivot to various functions within the realm of providing geriatric care in the clinic setting.
  10. Must be compliant with chart and EMR compliance policies.

  1. Highly organized, strong time management skills and flexibility to manage multiple timelines and activities.
  2. Strong leadership skills,
  3. Understands job responsibilities may extend beyond clinic hours for completion of chart notes and EMR in-basket items.
  4. Provides after hours call coverage when needed
  5. Willingness to play a key role in driving clinical initiatives related to the care of the aging population.
