The Licensed Clinical Social Worker (LCSW) works directly with both the medical providers and patients of Norwalk Community Health Center by providing direct interventions and psycho-education to patients referred for services. The LCSW provides behavioral health interventions through short-term, solution-focused interactions reflective of empowerment, both toward the patient and/or family, encouraging them to resume or regain management of their health and well-being.

- Trained in Behavior Modification, Parenting Skills, and Family Therapy

- Conducting intake assessments/discharge planning including imminent risk determinations
- Community outreach and committee participation as requested by management
- Insight into and understanding of working with substance abuse and pain management issues
- Experience Navigating Electronic Medical Records (EMR) or EHR.
- Ability to interact with a diverse population of people

Position Requirements:
- Capacity to develop and maintain relationships with the wider community of service providers
- Ability to make functional assessments of adults, youth, and families
- Knowledge of social skills assessment and interventions
- Must be able to use EMR scheduling/medical information system and other practice administration-related applications
- Completion of annual and ad hoc training as assigned and within identified timeframes
- Ability to diagnose and classify disorders using the DSM-5
