This position is 40% administrative and 60% direct patient care.  The incumbent will provide patient care in Veterans' homes within a 50-mile radius of The Tomah, Wisconsin campus.  Workload consists of a panel of 75-100 Veterans in the Tomah catchment area.  Home visits vary from 1 - 3 per day, depending on Interdisciplinary Team meetings.  Responsibilities include clinical oversight of all Home Based Primary Care satellites and three Nurse Practitioners, attendance at Interdisciplinary Team Meetings for La Crosse, Wisconsin Rapids, and Wausau, availability for clinical troubleshooting, and collaboration with the Home Based Primary Care Program Director.  Government-owned vehicle is available for travel.   

• $250,000 - $320,000 salary
