The primary purpose ofthisposition is to provide primary medical management in collaboration with the PACE Organization (PO) Physician(s).Performs assessments in collaboration with the physician and as needed. Develops and implementsappropriate plansof care to PO participants. Evaluates participant physical complaints and providesappropriate treatment. Provides participants and caregiver teaching and education. Functions as a member of the Interdisciplinary Team (IDT). Regularly informs the IDT of the medical condition of each participant,remainingalert to pertinent input from other team members, participants caregivers, as well as documenting changes in a participants medical record consistent with documentation policies.

What you will do:
- Provide primary medical coverage in collaboration with PO physicians as needed. Performs initial,semi-annualand annual assessments, performsadditionalassessments of participants as needed and implementsappropriate plansof care to PO participants.
- Participates in discharge planning for acute and long-term placement.
- Evaluate and treat participants during acute illness.Prudently prescribes medications,therapiesand other treatments for participants. Following consultation with the Primary Care Physician refers participants to medical specialist asindicated.
- Make nursing home/or home visits when needed.
- Work with Medical Director and Clinic Supervisor to formulate clinical policies,proceduresand standards of care.Assistswith the development of standards of care; performs on-going monitoring and evaluation of patient care practice and service delivery; provides guidance and training to staffregardingmedical and quality assurance issues.
- Advise the Medical Director and Primary Care Provider in ways and means toestablishbetter accountability of the PO services to participants and referral sources.Acts as resource during intake of new participants and in the day-to-day operation of the center programs.
Minimum Qualifications:
- Master's degree in Nursing
- Five (5) to seven (7) years of experiencerequiredpreferably in long-term care.
- Mustpossessa current, unencumbered, active license to practice in the state of PA.
- CDS Certification and DEA Registrationrequired.
- Board Certification by ANCC or AANP.
- Certification as a Nurse Practitioner with prescriptive authorityrequired.
- Current BLSrequired.
- A minimum of one (1) year working with the frail or elderly population stronglypreferred
- Must be action-oriented, be customer focused, have high decision quality, flexibility to adapt to ongoing change and have organizational agility.
- Must have a valid drivers license, proof of insurance and have means oftransportation
- Proficiencywith Microsoft product suite (MS Word, Excel, Power Point, etc.); basic knowledge and experience with electronic mail and calendaring system
- Position requires periodic travel within the Centers geographic region.
- Position requires frequent kneeling, crouching, reaching, lifting, sitting, walking,bendingand lifting. Must be able to push, pull, move and or lift a minimum of 20 pounds if applicable.
