Geriatric Medicine Job at ThedaCare, Inc. in Appleton, WI

Froedtert ThedaCare Health, Inc, Wisconsin's largest non-profit healthcare system, is seeking a Geriatric Physician to join our established team. You will be responsible for working with PALTC patients in community living facilities, developing relationships while administering high quality patient care. 

Position Details:

• Full-time or part-time options are available with flexible schedules!
• Locations: 1 Physician needed for Appleton and 1 Physician needed for Berlin/Oshkosh
• The team consists of 5 Physicians and 8 APC's
• 2-3 SNF's a day (patient schedule built in EPIC)
• On-Call by phone only: 1:11
• Patient census: 8-12
• Support staff includes RNs and schedulers who help to coordinate provider visits, communicate with facilities, and triage phone calls.
• 27 facilities total between SNF and ANF
• Large opportunies for growth such as Medical Director roles in the nursing homes that we serve

Will accept Internal Medicine, Family Medicine or Geriatric trained.

• Income Details: Guaranteed base salary plus value-based incentives
