Pay Rate: $45.00-51.47 hourly

Minimum Requirement: 

• Doctor of Physical Therapy from CAPTE accredited program.
  
• 2 -3 years' physical therapy provider experience.
  

License & Certification: 

• Colorado Physical Therapy License, or ability to obtain within 90 days of hire.
  
• American Heart Association Basic Life Support Provider (CPR/AED).
