Partnership Health an outpatient Family Practice open only to the employees of the Franklin township school system is seeking a dedicated and compassionate Family Nurse Practitioner (FNP) to join our healthcare team. The ideal candidate will provide comprehensive care to patients across various age groups, focusing on preventive health measures, chronic disease management, and overall wellness. The FNP will work in an outpatient setting and collaborate with a multidisciplinary team to deliver high-quality patient care.

Schedule will be 3 - 10 hour shifts during the week and every other weekend Sat and Sun from 8am-1pm

Responsibilities

• Conduct thorough physical examinations and assessments of patients.
• Develop and implement individualized care plans based on patient needs.
• Educate patients on health maintenance, disease prevention, and treatment options.
• Collaborate with healthcare professionals to ensure continuity of care.
