Our Licensed Clinical Social Worker's (LCSW) play a crucial role in identifying and addressing the biopsychosocial needs of members with highly complex medical and mental health conditions and creating a viable treatment plan to move members towards optimal physical and mental health. Armed with a graduate degree and clinical licensure that prioritizes prevention and empowers the member's self-management, along with high levels of compassion and strong interpersonal skills: LCSW's help members better navigate medical, behavioral health and life circumstances to meet their needs.

 

Responsibilities

The CRC LCSW will provide support to the outpatient dual-risk capitated members accessing our network Hospitals. The LCSW will provide community resources and coordinate resources as indicated, collaborating directly with primary care offices, MSO’s, CRC and hospital care management, specialty/hospital-based physicians, and others.

 

The LCSW will: advocate for patients; empower patients to become effective advocates for their own needs and the needs of their families; identify and support patients overcoming obstacles related to treatment plans and other medical, social, and/or behavioral health services; help patients secure necessary and appropriate medical, social, and/or behavioral health services.

 

The LCSW is responsible, along with other members of the Medical Management Team, for contributing to the clinical, quality, financial, and member satisfaction outcomes.

 

Qualifications

  • Required: Master’s degree in social work (MSW) from an accredited institution.
  • Fully and independently unencumbered California LCSW license.
  • Minimum 5 years’ combined experience post degree (3 years internship/pre-licensed and 2 years post license) in healthcare, behavioral health, psychiatric and/or substance abuse health care setting.
  • Knowledge of and experience in inpatient and/or ambulatory settings.
  • Experience and working knowledge of hospice and palliative care.
  • Knowledge of DSM-5 or most current diagnostic edition and Milliman (MCG) Care guidelines.
  • Ability to analyze specific utilization challenges, plan and implement solutions which directly influence quality of care.
  • Understanding of health plan benefits (healthcare and behavioral health structures), psychiatric/medical terminology, voluntary and involuntary treatment parameters.
  • Possess the strong interpersonal skills required to build trusting, supportive, effective relationships with patients. This includes high levels of empathy, respect for patients/families, genuineness, acceptance, and concern for well-being.
  • Courteous, cordial, cooperative, and professional interaction with diverse groups of co-workers, hospital team members, external business partners and the community.
  • Willingness to make face-to-face or virtual visits independently with members in a variety of safe settings: member home, provider office, Skilled Nursing Facility, etc.
  • Ability to travel to multiple locations within greater Los Angeles County, CA; valid California driver’s license and dependable automobile that is insured.
  • Extensive familiarity with available community resources throughout the state of California to meet the needs of Medi-Cal and Medicare members enrolled in the CRC risk-based Medi-Cal health plans, Medicare (MA) and D-SNP Medicare Advantage Plans.
  • Independently computer literate: proficient with Microsoft 365 Suite including but not limited to Teams, Word, Excel, Power Point, Care Management software and web-based applications.
  • Strong written and verbal communication.
  • Highly organized and able to work independently as well as collaboratively under hybrid working conditions (your home and field locations).
  • Bilingual (Spanish/English) is preferred.
