Patient Focused RN Care Manager Opportunity

Job Description

As an RN Care Manager for PaceKC, you will be part of a team of caregivers to provide care for the members of our community that need it the most. The RN Care Manager works as a member of the interdisciplinary team to assess, plan, implement and evaluate care provided in the patient/participants' place of residence. This nurse will actively participate in the coordination of all aspects of patients' care, in accordance with current professional standards and practice. You play a vital role for the patient-centered medical team in providing quality, patient-centered care.

In your role you will:

  1. Provides patient/caregiver education and training specific to identified needs.
  2. Collaborates in the provision of care, treatment and services with the interdisciplinary team to meet patients’ needs and maintain continuity of care between disciplines, programs, services and with other organizations as needed.
  3. Participates in agency activities including interdisciplinary team meetings, staff meetings, and other committee meetings as assigned and educational opportunities.
  4. Collaborates with the physician(s) and acts as liaison for the care coordination of the designated case load.
  5. Assesses patient needs and provides services within the scope of practice and based on an individualized plan of care appropriate to the patient’s needs, strengths, limitations and goals.
  6. Communicates/collaborates with contracting providers regarding services provided.
  7. Evaluates care, treatment, and services to include active participation in interdisciplinary team meetings.
  8. Assesses discharge and/or transfer needs and facilitates the process to ensure that continuity of care, treatment and service is maintained.
  9. Follows policies and procedures in the assessment, planning, intervention, evaluation and documentation of patient and family care.
  10. Performs clinical duties within the nursing scope of practice.
  11. Documents in the electronic record within stated standards provide complete and accurate supporting documentation.
  12. Participates in the agency performance improvement program.
  13. Participates in the surveillance, prevention and control of infection.
  14. Participates in environmental safety and equipment management procedures.
  15. Utilizes information management to support decision-making to improve patient outcomes, improve healthcare documentation, assure patient safety and improve performance in patient care, treatment and services.


Successful Candidates will have:

  1. Graduated from an accredited school of practical nursing.
  2. Current unrestricted State of Kansas License to practice as a Registered Nurse .
  3. A minimum of 1 year professional medical-surgical experience
  4. A minimum of 1 year's experience working with the geriatric population.
  5. Knowledge of professional nursing theory, standards of nursing practice, scope of practice and relevant statutes and regulations
  6. Ability to prioritize work, meet deadlines, handle multiple projects, pay strong attention to detail and accuracy and work under pressure in a cooperative work environment.
  7. Must possess a valid Kansas Driver’s License, in force automobile liability insurance and maintain a Motor Vehicle Record (MVR) that meets organization’s insurability standards as defined by Midland policy.
