This is an Illinois-based, comprehensive primary care provider with three locations (Peoria, Galesburg, Carbondale). We are not affiliated with any hospital or health system but work as a partner with the healthcare ecosystem in each location to navigate the healthcare needs, referrals, specialty care, and hospitalizations of our patients. Our mission is to deliver exceptional care through a comprehensive primary care model that makes it easy, convenient, accessible, and affordable for all. This includes serving the entire community, accepting most insurance, and providing services regardless of ability to pay.

We have exciting opportunities for Family Practice Physicians that share our vision of transforming how primary care is delivered to the communities we serve. We are a non-profit Federally Qualified Healthcare Center that focuses on hospitality and taking care of as many of the patient’s needs under one roof as possible. Our comprehensive model has grabbed the attention of patients that feel lost in the ‘big box’ healthcare systems and allowed the growth of two additional locations in our first two years.
