Nurse Practitioner – Primary Care (All Ages)

Community Health Net is seeking a compassionate and skilled Nurse Practitioner to join our team and provide comprehensive primary care services to patients of all ages. What You’ll Do

As a key member of our care team, the Nurse Practitioner will:

• Provide primary care services including diagnosis, treatment, preventive care, care coordination, and health maintenance for patients of all ages.
• Deliver care in a compassionate, respectful, and culturally competent manner.
• Support patients in achieving their health goals through personalized treatment plans and education.

Qualifications

• Current and valid Pennsylvania Nurse Practitioner and Registered Nurse licenses
• DEA license (preferred)
• Successful completion of required credentialing process
• Familiarity with Electronic Health Records (EHR); NextGen experience a plus
• Proven ability to provide quality, comprehensive outpatient medical care
• Outpatient healthcare experience preferred
