About the Role:

We’re looking for an experienced Advanced Practice Clinician (APRN or PA) to join our care team in a hybrid role serving Springfield and East St. Louis, IL. You’ll provide a mix of virtual and in-person care when needed, to be focused on chronic disease management, acute concerns, behavioral health, and preventive care, including sexual and reproductive health.

Responsibilities:

Manage chronic conditions (e.g., diabetes, hypertension, CKD)

Address acute issues and behavioral health needs (e.g., anxiety, depression, SUD)

Deliver preventive care and wellness support

Collaborate with an interdisciplinary care team

Support development of clinical programs and care initiatives

Requirements:

Licensed APRN or PA in Illinois

3+ years clinical experience in primary or chronic care

Tech-savvy with comfort in virtual care platforms

Willing to travel in-region as needed
