Geriatric Medicine Job at Tidalhealth in Salisbury, MD

TidalHealth on Maryland's east coast is seeking a dedicated and compassionate Geriatric Nurse Practitioner (NP) to join our patient-centered team focused on the health and well-being of older adults. This role offers the opportunity to make a meaningful impact on the lives of geriatric patients through direct clinical care, chronic disease management, and coordinated support services.

Key Responsibilities:

• Provide comprehensive primary care and chronic disease management to geriatric patients in outpatient, home, or long-term care settings

• Conduct routine and acute assessments with a focus on prevention, wellness, and individualized treatment plans

• Collaborate with physicians, specialists, caregivers, and interdisciplinary team members to coordinate care

• Educate patients and families about aging-related conditions, medications, and lifestyle modifications

• Document and maintain accurate electronic medical records in accordance with regulatory standards

• Participate in care team meetings and quality improvement initiatives

Qualifications:

• Current and unrestricted RN and NP license in the state of MD

• Certification as an Adult-Gerontology Nurse Practitioner (AGNP) or Family Nurse Practitioner (FNP) with experience in geriatrics

• Minimum of [1–3] years of NP experience preferred, particularly with elderly populations

• Strong clinical assessment and diagnostic skills

The approved hiring range is minimum $120,000 and maximum $140,000.
