Job Details:- Working in the town of Bay City, MI
- 15 -20 patients per day
- This is a full-time position
- In addition to nurse practitioner (NP) candidates, physician assistants (PA) are welcome
Compensation and Benefits: Our compensation is a productivity-based model (Fee for Service). Expected compensation is estimated income range of $130,000 to $154,000 annually with no cap on productivity bonus income potential. This full-time role is benefit eligible.
Responsibilities:- Drive high-quality, safe, and cost-effective patient care in post-acute carecommunities
- Inpatient rounding in skilled nursing facility, inpatient rehabilitation
- Collaborate with other clinician interdisciplinary teams and follow CMSguidelines
- Work closely with hospice, palliative care, and home care agencies for safepatient transitions
- Ensure timely documentation and billing through our EMR (Gehrimed) meetingMedicare guidelines
Qualifications:
- Board certified FNP, AGACNP, ACNP, or AGPCNP
- Willing to train experienced candidates as well
- Licensed in state
