Position Summary:
myPlace Health is a mission-driven organization that delivers compassionate, comprehensive care for older adults. We are seeking a dedicated Staff Physician with a strong background in Geriatric Medicine and a passion for caring for frail and elderly populations. This is a full-time, onsite role located in Greater/Downtown Los Angeles.
This position plays a key role within our PACE (Program of All-Inclusive Care for the Elderly) program, which integrates medical, social, and behavioral care into a seamless, person-centered approach.
Key Responsibilities:
Provide comprehensive medical care for a panel of geriatric participants, emphasizing prevention, chronic disease management, and functional wellness
Collaborate with interdisciplinary teams to coordinate individualized care plans
Conduct initial, annual, and routine health assessments
Participate in after-hours and on-call coverage rotation as required
Maintain high-quality documentation in the EMR system
Participate in quality improvement initiatives and case conferences
Promote a respectful, inclusive, and equitable environment for staff and patients
Required Qualifications:
Doctorate degree (MD or DO)
Active Medical Doctor (MD) or Doctor of Osteopathic Medicine (DO) license in California
Active CDS (Controlled Dangerous Substances) registration
Active DEA (Drug Enforcement Administration) license
Valid Driver’s License (CA)
Board Certified or Board Eligible in Internal Medicine, Family Medicine, or Geriatrics
Current BLS or ACLS certification (ARC BLS, AHA BLS, or AHA ACLS accepted)
Experience with frail or elderly populations in primary or geriatric care
Prior PACE experience (must be noted on resume)
Preferred Qualifications:
Bilingual in Spanish (read, write, speak fluently)
Strong knowledge of interdisciplinary care models and elder care best practices
Commitment to health equity and culturally competent care
Excellent communication and documentation skills
