Position Overview HarmonyCares Medical Group is seeking a highly skilled and compassionate Nurse Practitioner to join our dynamic team of healthcare professionals in the Cleveland area. This pivotal role involves providing comprehensive primary care to high-acuity geriatric patients, empowering them to live healthier, fuller lives within their own familiar environments. Key Responsibilities

  • Manage a small patient panel, dedicating up to one hour of personalized time per patient to ensure comprehensive and unhurried care.
  • Provide a wide range of primary care services, including medication management, proactive symptom control, chronic disease management, and sensitive end-of-life care.
  • Collaborate seamlessly with a multidisciplinary team, including physicians, medical assistants, and other healthcare professionals, to ensure coordinated and integrated patient care.
  • Monitor patient outcomes diligently, adjusting care plans as needed to optimize health and well-being, and providing ongoing support to patients and their families.
  • Conduct regular home visits to assess patients' needs in their natural environment and deliver direct, hands-on care.
  • Maintain accurate, timely, and thorough medical records and documentation in compliance with all regulatory standards.

Qualifications

  • Master's degree in Nursing Practice (MSN) or Doctorate of Nursing Practice (DNP) from an accredited program.
  • Current Board Certification as a Nurse Practitioner (NP-C).
  • Minimum of 2 years of clinical (NP) experience, preferably within primary care, geriatrics, or home health settings.
  • Strong clinical assessment skills, including proficiency in physical examination, diagnostic testing interpretation, and medication management.
  • Exceptional communication and interpersonal skills, with the ability to build rapport and effectively collaborate with patients, families, and healthcare team members.
  • Ability to work independently with a high degree of autonomy, as well as collaboratively within a supportive team structure.
  • Valid driver's license
