Southern Illinois Healthcare (SIH) is seeking a LCSW to join their established & collaborative behavioral health team. Our LCSWs are responsible for the overall operation of social service activities within the emergency department.
- Multiple work locations and departments available
- Preferably already living in/near southern Illinois
- Should have knowledge of southern Illinois' area resources
- Required: Masters degree in Social Work
Responsibilities include:
- Safety
- Manages processes effectively in regard to employee/patient safety.
- Record Keeping
- Maintains all required records, reports, statistics, logs, files, and other documents as required.
- Process Improvement
- Promotes a culture of process improvement by participating in unit/department-based programs that support the system’s process improvement goals.
- Actively participates on system-wide or hospital-based teams as needed.
- Role Specific Responsibilities
- Completes initial and ongoing psychiatric assessments for identified patients who present with mental health issues.
- Through the use of high risk screens and communication with providers, determines and refers appropriate cases for admission, transfers to psychiatric hospital, or community crisis support as it relates to discharges.
- Identifies indicators of abuse or neglect and intervenes per regulatory requirements.
- Coordinates discharge plan in conjunction with case manager, patient, family/significant others.
- Presents options to patient, family/significant others to address their needs and assisst them with decision-making process.
- Coordinates information and referrals for financial and community resources in conjunction with case manager.
- Coordinates patient care with inpatient/referral placement and communicates issues assessed and possible interventions.
- Maintains confidentiality, is courteous to all patients, families and all other hospital staff-addresses and follows-up on departmental patient concerns.
- Provides excellent customer services. Knows and practices hospital identified customer service behaviors. Identifies responsiveness to customer needs as a personal priority.
- Supports the importance of continuous quality improvement. Participates in department quality improvement projects. Participates in hospital-wide quality improvement projects, as needed.
- Supports Service Excellence initiatives contributing to the organization, becoming a top performer in quality care, patient safety and patient satisfaction strategies. Understands and applies techniques to support the provision of population based appropriate care utilizing Joint Commission standards, CMS Core Measure requirements, and other best practices.
- Is a self-learner and fosters learning in others. Pursues continuing education in area of expertise/leadership. Achieves and maintains hospital-wide and department-identified competencies.
- Participates in educational offerings provided by the hospital; attends all mandatory training programs and actively participates in departmental staff meetings.
